Examples of emails you may receive from PKB on behalf of My Health and Care Record

Am I already registered for My Health and Care Record?

You will receive an email notifying you that we (your GP practice) are requesting access to contribute to your personal health record. This is because a key principle is that you have control over your healthcare data and who can or cannot see the data held within My Health and Care Record. The email will look like this:

If I already have a My Health and Care Record or a PKB record with an email address that I have not activated

You may already have a My Health and Care Record or a Patients Know Best record as one may have been created for you as part of a hospital visit. If your email address is associated with that record, you will receive an email that looks like this:

If my record (My Health and Care Record or PKB) is not created in Sussex

A record may have been created when you were admitted to a hospital outside of Sussex.  You will be asked to accept the request and the email will look like this:
