Personal Heath Record

We are pleased to offer patients within the Preston Park PCN catchment area a new online service that gives you secure access to your personal health record. It’s designed to improve your experience and access to NHS services and information. The practices involved are Warmdene Surgery, The Haven Practice and Stanford Medical Centre.

Once you have registered you will simply log in to view your healthcare information or use it to understand and monitor your condition.

This record, which contains your health information, is yours and yours alone and is a simple and secure way to add, manage and organise your health and wellbeing information.  Please keep it safe and secure at all times.

Step 1: Register

Your record has already been created by the system at your GP (EMIS) and we will use your verified email address, where provided, to send you an invitation to register. Click here [joinpkb.com] to complete registration if you have received an invitation. It takes no more than 2 minutes to register.

If you have not received an invitation but would like to register please speak to a member of reception staff during your next appointment – they will also be happy to get you started.

Step 2: Access your health information

Once you have registered you will then have access to your record and all the benefits of having it in one place.  Please click here to login into your record.

When you have access to your record you can:

  • Give clinicians access to your record
  • Give carers and/friends and family members access to your records
  • Book appointments
  • Connect your wearable devices to your record so that healthcare teams can see your data

When you are in your record you may find that some areas are unpopulated which means that those areas are not yet accessible to you.  When they do we will let you know that this has happened and how they can work for you.
